You may submit a claim by filling in the online form at the CNPD -  National Commission for Data Protection’s website. 

When submitting a claim, you must briefly describe the facts. Documents cannot be sent, you should only indicate if there is evidence of the facts. If necessary, the CNPD will ask you to send the proof documents afterwards.
